A CASUALTY consultant and a GP both sent a woman home to die after failing to diagnose that she had a blood clot in her lungs, an inquest heard.

Jayne Hughes, 44, was rushed into hospital by ambulance after having a dizzy spell and falling downstairs at her home in Lydney.

But although she was breathless with high blood pressure, a fast pulse and a low oxygen count she was discharged with painkillers by consultant Delia Parnham-Cope after just an hour-and-a-half.

Later that day her worried mum took her to her local GP surgery because her condition was worsening- but locum doctor Meenakshi Raina also failed to pick up on the true cause of her symptoms and gave her steroids.

Mum of two Ms Hughes returned home in a state of near collapse and died in bed the following night, an inquest was told.
